For New Hires at The Summit, please complete the following so we can get you set up for your first day.
Fill Out A new hire Form
Needed documents mentioned inside the New Hire Form will be:
  • a W4 (link to this document will be found in the form below)
  • a Tax Withholding form (link to this document will be found in the form below)
  • an I9 (link to this document will be found in the form below)
  • a copy of your Driver's License (needed for your I9)
  • a current copy of your Social Security Card or Birth Certificate (needed for your I9)
  • Direct Deposit Information (link to this document will be found in the form below)
